Under the Home and Community-Based Services (HCBS) Medicaid Intellectual Disability (ID) Waiver, GROW-AN provides high-quality Supported Community Living (SCL) services in Linn and Johnson County for adults aged 18 and older.

Our agency is committed to delivering person-centered, compassionate care that fosters personal growth, independence, and dignity. Through individualized support plans, we empower each person we serve to navigate daily living, achieve their goals, and fully participate in their community, all while ensuring compliance with state and federal regulations.

• We offer 24- hour daily support tailored to each individual. 
• Our homes are located in residential settings near city parks, shopping centers, and bus routes. 
• Each site houses 4 individuals.

• We provide flexible, hourly support in your home or out in the community—offering assistance wherever you need it most, including reliable transportation to appointments, activities, and more.

• At GROW-AN, we love helping people feel inspired, informed, and ready to make a difference. Our motivational speaking and training events are all about real talk, real stories, and real strategies.
• Whether you're a caregiver, team leader, or just someone who wants to learn more, our sessions are engaging, down-to-earth, and packed with tools you can actually use.
